2020 registration form with built-in math - Important: For the form with math to work properly, you MUST download (save) it to your computer and then open the file on your computer. If you attempt to complete the form within your web browser, it will not work properly.
2020 registration form that you calculate yourself - print and complete by hand.

2. Mail registration and payment to the address below, (also listed on the form), or pay via PayPal (see the button below).

Important Payment Notes

A $15 fee will be added to all PayPal and credit card transactions to offset transaction costs.

CANCELLATION POLICY & REFUND POLICY

If you cancel BEFORE May 31, a $50 processing fee will be withheld from your refund.
If you cancel AFTER May 31, $100 deposit will be withheld from your refund.
No refunds after June 30.
